Marjinal Ülser Perforasyonunun Falciform Ligaman ile Onarımı: Olgu Sunumu

Abstract

Marjinal ülserler, gastroenterostomi hattında ortayaçıkan ülserlerdir. Biz, bu yazıda, peptik ülser hastalığıiçin 5 yıl önce cerrahi öyküsü olan ve peritonit ilebaşvuran 45 yaşındaki erkek hastada ortaya çıkanmarjinal ülser perforasyonunu sunduk. Hastanın nonsteroidal anti-inflamatuar ilaç kullanımı ve sigaraiçiciliği öyküsü vardı. Hastanın özgeçmişinde bilinenbaşka bir hastalığı yoktu. Hasta acil operasyona alındı vegastrojejunostomi hattının anteriorunda afferent ansta1x1 cm perforasyon ve peritoneal boşluğun safralı sıvıile yaygın kirlenmesi mevcuttu. Hastaya nadir biruygulama olan falciform ligaman ile onarım yapıldı.Postoperatif süreçte hastanın herhangi bir problemiolmadı. Falciform ligaman ile perforasyon onarımı,omentopeksiye alternatif olarak ucuz ve kolayuygulanabilir bir yöntemdir

Marginal ulcers occur along the gastroenterostomyline. We report a case of marginal ulcer perforation ina 45-year old male presented with peritonitis and abackground of surgery for peptic ulcer disease 5 yearsago. The patient had a smoking history and use of nonsteroidal anti-inflammatory drug. There was no otherdisease known in the patient's past. The patient wastaken to emergency operation and there was 1x1 cmperforation on the afferent loop of of the anteriorgastrojejunostomy line and extensive staining of theperitoneal cavity with bilious fluid. Repairs were donewith the falciform ligament which is a rare practice.The patient did not have any problems in thepostoperative period. The repair of the perforationwith the falciform ligament is a cheap and easilyapplicable method that is an alternative toomentopexy
